We will record your incentive submission in our system, and at the end of the quarter, we will processing your incentive. Please note, before we can issue payment, the following must be submitted:

  • The form, complete with department information and product serial numbers.
  • Photo evidence of the product on the apparatus.
  • A completed W-9 (or W-8); only one needs to be on file for 2020. This can be emailed to [email protected].
